兰索拉唑与奥美拉唑治疗老年酒精型消化性溃疡的疗效

Efficacy of Lansoprazole and Omeprazole Therapy on the Elderly Alcoholic Peptic Ulcer

目的观察兰索拉唑与奥美拉唑治疗老年酒精型消化性溃疡患者的临床疗效及安全性。方法选择2010年6月至2011年6月在中山市三角医院就诊的酒精型消化性溃疡老年患者90例,按照随机数字表法将患者分为治疗组45例,口服兰索拉唑30 mg治疗,每日1次;对照组45例,口服奥美拉唑20 mg治疗,每日1次。观察两组患者溃疡愈合率、幽门螺杆菌(Hp)根除率、不良反应等情况。结果用药第1日和第7日时上腹疼痛消失率在治疗组分别为84.4%和95.6%,对照组分别为55.6%和82.2%,两组比较差异均有统计学意义(χ2=8.942,4.050,P<0.05);经胃镜检查显示,治疗组和对照组的溃疡愈合率分别为80.0%和55.6%,总有效率分别为97.8%和80.0%,比较两组消化性溃疡治疗后的愈合情况,治疗组显著优于对照组(Z=-2.854,P<0.05);两组Hp检测阴性率分别为88.9%和71.1%,比较差异有统计学意义(χ2=4.444,P<0.05)。所有患者均未发现明显的不良反应。结论兰索拉唑和奥美拉唑治疗老年酒精型消化性溃疡均安全有效,但兰索拉唑在缓解症状、促进溃疡愈合及根除Hp等方面均优于奥美拉唑。

Objective To observe the efficacy of lansoprazole and omeprazole in treatment of alcoholic peptic ulcer in the elderly.Methods Ninety elderly patients with alcoholic peptic ulcer were selected from June 2010 to June 2011 in Zhongshan Triangle Hospital and divided according to the random number table into treatment group with 45 patients-treated oral lansoprazole 30mg everyday,and control group with 45 patients-treated with omeprazole,20mg every day.The eradication rate of Helicobacter pylori（Hp）,the healing rate of peptic ulcer and adverse reactions were observed.Results At the 1st and the 7th day after giving medication,abdominal pain disappearance rate in the treatment group were 84.4% and 95.6%,and were 55.6% and 82.2% in the control group,there were significant differences between the two groups（χ2 = 8.942,4.050,P 0.05）.By endoscopy,the ulcer healing rate of the two groups group was 80.0% and 55.6% respectively,the total effective rate of thetwo groups was 97.8% and 80.0% respectively.Peptic ulcerhealing after treatment were compared between the two groups,it was better in the treatment group than that in the control group（Z =-2.854,P 0.05）.The H pylori negative rate in the two groups was 88.9% and 71.1%,respectively.There were significant difference between the groups（χ2 = 4.444,P 0.05）.No adverse effects were observed in all patients.Conclusion Lansoprazole and omeprazole are both safe and effective in treating alcoholic peptic ulcer in the elderly,while lansoprazole is better than omeprazole in improvingsymptoms,curing ulcers,and eradicating Hp.
