人附睾蛋白4在卵巢上皮癌诊断中的应用价值 被引量:5

Detection of Serum Human Epididymis Protein 4 and Its Significance in Diagnosis of Ovarian Cancer

摘要 目的采用ROC曲线评价人附睾蛋白4在卵巢上皮癌诊断中的准确性及阴、阳性预测值。方法收集112例经病理学确诊的卵巢上皮癌患者,健康对照组为100例年龄和性别相匹配的体检人群。所有对象均行人附睾蛋白4浓度定量检测。采用MedCalc软件绘制ROC曲线,计算AUC面积,并据此评价人附睾蛋白4在卵巢上皮癌排除诊断中的准确性和阴阳性预测值。结果与正常对照组相比,卵巢上皮癌组的人附睾蛋白4浓度明显升高,差异有统计学意义(P<0.05)。以0.61μg/ml作为卵巢上皮癌的诊断临界值,此时的AUC面积为0.907。以0.20μg/ml作为卵巢上皮癌排除诊断临界值时的阴性预测值为95.2。结论人附睾蛋白4检测是卵巢上皮癌诊断和排除诊断的较理想指标。人附睾蛋白4检测诊断卵巢上皮癌理想临界值为0.61μg/ml,但作为卵巢上皮癌排除诊断指标时的理想临界值为0.20μg/ml。 Objective To detect the level of serum human epididymis protein 4 (HE4) in the patients with ovarian cancer and approach the significance of serum HFA in the diagnosis of ovarian cancer. Methods Total 112 cases of epithelial ovarian cancer confirmed by pathobiology examination and 100 normal subjects were enrolled in this study. ELISA method was used to de- tect the levels of serum HFA in both groups. The value of the serum HFA level in the diagnosis of ovarian cancer was evaluated by ROC analysis. Results The serum HE4 level in the ovarian cancer group was higher than that in the healthy individuals ( P 〈 0.05). When the critical value was defined as O. 6lug/m1, the area under the curve (AUC) was 0. 907 with a sensitivity of 88.2% and specificity of 92.4%, respectively. Conclusion The HE4 serum has important significance in the diagnosis of ovari- an cancer, especially, its sensitivity and specificity are all better than other traditional tumor marker in the diagnosis o f early ovar- ian cancer,
