摘要
目的评价虹膜缝线固定折叠人工晶状体的临床效果。方法随机数字法将无晶状体眼62例(63眼)分成A、B两组。A组行虹膜缝线固定植入折叠人工晶状体,B组行巩膜缝线固定折叠人工晶状体,术后3个月比较两组视力和屈光状态。术后4周内,每周观察前房闪光。结果术后3个月,A组裸眼视力为(0.43±0.19)优于B组的(0.35±0.18),P〈0.05,差异有统计学意义;最佳矫正视力(BCVA)两组差异无统计学意义;术后裸眼视力:A组有38.7%(12/31)高于术前最佳矫正视力(BCVA),B组有12.5%(4/32)高于术前最佳矫正视力BCVA;A组有29%(9/31)较术前最佳矫正视力降低,B组有56.3%(18/32)较术前最佳矫正视力降低。显然验光球镜,A组成近视化趋势(-0.47±0.58)D,B组成远视化趋势(0.50±0.43)D;显然验光柱镜,A组(0.84±0.53)D小于B组(1.23±0.70)D,差异有统计学意义;角膜散光,两组差异无统计学意义;A组与B组相比,有较少并发症发生。结论短期观察,虹膜缝线固定折叠人工晶状体视力效果、屈光效果优于巩膜固定折叠人工晶状体。
Objective To evaluate the clinical effects of iris- fixated foldable lens. Methods 62 patients (63 eyes) who had insufficient posterior capsular support for intraocular lens had been divided into two groups randomly, group A underwent iris-fixated foldable lens implant, group B underwent scleral-fixated foldable lens implant, 3 months after operation, visual acuity and refractive status were compared ; the first 4 weeks postoperatively, dyndall effect was observed every week. Results 3 months after operation, the UC- VA was better in group A (0.43 ±0.19)than in group B (0.35 ±0.18), but the BCVA had no difference between the two groups; The postoperative UCVA: 12 of 31 eyes (38.7%)in group A were better than pre- operative BCVA, and the percentage in group B was 12.5% (4/32) , however, 9 of 31 eyes (29%) in group A and 18 of 32 eyes (56.3%) in group B were worse than preoperative BCVA; postoperatively, the sphere in group A was minus ( -0.47D±0.58D), and that in group B was plus (0.50D ±0.43D), the cylinder in group A (0.84D ±0.53D) was smaller than in group B ( 1.23D ±0.70D), the difference was statistical significant, but the corneal astigmatism had no difference between the two groups; there were less complications in group A than in group B. Conclusion In short term, compared with group B, the visual results and refractive effects in group A were better.
