补肾健脾疏肝方治疗桥本氏甲状腺炎的临床研究

Clinical studies of reinforcing kidney and strengthening spleen and soothing liver prescription on treating Hashimoto's thyroiditis

目的观察自拟补肾健脾疏肝方联合小剂量优甲乐治疗桥本氏甲状腺炎继发亚临床甲状腺机能减退患者的疗效。方法将50例此类患者随机分为两组,对照组口服左甲状腺素钠每天25ug,治疗组在此基础上加服中药补肾健脾疏肝方。两组均以3个月为一疗程,共治疗二个疗程。结果在中医证候总疗效,中医证候积分神疲乏力、畏寒肢冷、便溏、急躁易怒症状以及甲状腺功能指标:促甲状腺激素(thyroid stimulating hormone,TSH)和免疫指标:血清甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPOAb)、甲状腺球蛋白抗体(thyroglobulin antibody,TgAb)方面,治疗组均明显优于对照组(P<0.05)。结论补肾健脾疏肝方治疗桥本氏甲状腺炎继发亚临床甲减有较好疗效。

Objective To probe into the efficacy of the ＂reinforcing kidney and strengthening spleen and soothing liver prescription＂ in treating the Hashimoto＇s thyroiditis concomitant subclincal hypot- hyroidism. Methods 50 diagnosed ttashimoto＇s thyroiditis concomitant subclinical hypothyroidism patients were randomly divided into two groups on the principle of randomized control in clinical trials. The control group was treated by taking levothyroxine sodium （L-T4Na） orally （25 ~g per day） ; the treatment group was treated by taking reinforcing kidney and strengthening spleen and soothing liver prescription orally one dose per day as supplement to the L-T4Na taken orally. Two courses of treatment each lasting three month were given to both groups. Results In terms of TCM syndrome efficacy assessment, TCM syndrome score assessments such as the lassitude, intolerance to cold, cold hands and feet, loose stool, irascibility, and thyroid stimulating hormone level as well as thyroid autoantibodies TPOAb and TGAb, when compared be- tween the groups, the treatment group was associated with better efficacy （P 〈 0. 05）. Conclusion rein- forcing kidney and strengthening spleen and soothing liver prescription proved to have an excellent treatment effect to the Hashimoto＇s thyroiditis concomitant subclinical hypothyroidism.
