三维有限元模型分析OSAHS患者下颌不同前伸位置引起髁状突位置变化及应力分布的研究 被引量:1

Influence of Different Mandibular Protrusion Position on Stress Distribution and the Condyle Changing through Three-dimensional Finite Element Model in OSAHS Patients

摘要 目的研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者口腔矫治器治疗时,下颌逐步前伸对患者髁状突的位置变化及应力分布的影响。方法对1例OSAHS患者颞颌关节及上气道行薄层CT扫描,获得该患者DICOM格式的图像信息,采用Mimics、Imageware及Ansys有限元分析软件建立三维有限元模型,然后通过加载下颌骨逐步前伸,观测髁状突的位置变化及应力分布规律。结果下颌逐步前伸不同量时,髁状突位移及应力都发生一定的变化,各部位位移呈增大趋势,而应力在下颌骨前伸68%时,髁突各部位整体的应力值最小。结论通过观测OSAHS患者髁状突的位置及应力变化,从颞颌关节方面为临床利用口腔矫治器治疗OSAHS提供理论基础。 Objective To quantify the condyle position and stress distribution as the mandible gradually for-ward in patients with OSAHS. Methods A three - dimensional finite element model of the mandible and temporomandibular joint was established using CT, imaging Mimics, Imageware and Ansys software. The change of condylar position and stress distribution were observed through gradually loading the protrusive man-dible. Results As the mandible gradually forward at different levels, the condylar position and stress distri-bution underwent corresponding change and various parts of the displacement was increased. When the mandi-ble protrusion was 68%, condylar stress values of the various parts got the minimum. Conclusion By obser-vation condyle position and stress changes in patients with OSAHS, a theoretical basis in temporomandibular joint will be provided for the clinical use of oral appliances for the treatment of OSAHS.
