右美托咪定用于大面积烧伤患者早期镇静镇痛的临床观察

Clinical Observation on Dexmedetomidinene Used for Early Sedation and Analgesia in patients with Large-Area Burns

目的观察右美托咪定用于大面积烧伤患者早期镇静镇痛的有效性和安全性。方法将40%～80%大面积烧伤患者22例随机分为3组。D1组、D2组给予盐酸右美托咪定注射液,静脉泵注,泵注速率D1组为0.2μg/(kg.h),D2组为0.4μg/(kg.h);P组给予0.9%氯化钠注射液泵注,泵注速率为10 mL/h。观察患者镇痛前、镇痛30 min的生命体征及镇痛评分。结果 D1组及D2组与P组的生命体征无明显差异。D1组、D2组与P组的镇痛评分有显著性差异。结论静脉泵注0.2～0.4μg/(kg.h)右美托咪定对大面积烧伤患者有明显的镇静镇痛效果,且对呼吸循环影响不明显。

Objective To observe the efficacy and safety of dexmedetomidine used for early sedation and analgesia in the patients with largearea burns. Methods 22 cases of 40% -80% largearea burns were randomly divided into three groups. The group D1 and D2 were given Dexmedetomidine Injection by intravenous pump infusion with a pumping rate of 0.2 μg/（kg · h） in the group D1 and a pumping rate of 0. 4 μg/（kg · h） in the group D2. The group P was given 0. 9% normal saline by intravenous pump infusion with a pumping rate of 10 mL/h. The vital signs and the sedation scores were observed before and after analgesia. Results The vital signs in the group Dl, D2 and P had no obvious difference. However, the sedation scores showed significant difference among the group D1, D2 and P. Conclusion Dexmedetomidine by intravenous pump infusion with a pump rate of 0. 2- 0.4 μg/（kg · h） has significant sedative and analgesic effect on the patients with largearea burn, meanwhile, its influence on respiration and circulation is unconspicuous.
