摘要
目的探讨脑出血患者血压水平与预后的关系。方法回顾性分析2008—2011年我科收治的经颅脑CT检查证实的急性首次脑出血患者共130例,分别收集患者自然信息、首次血压记录、出血量、中线移位情况及是否破入脑室、首诊时格拉斯哥昏迷评分(Glasgow coma scale,GCS)、住院中肺部感染发生情况及3个月随访时的改良的Ranking量评分(modifiedranking scale,MRS)等资料,进行单因素分析及多元线性回归分析。结果研究结果表明年龄、GCS评分、脑室积血、舒张压、中线移位、肺部感染是影响预后的主要因素,血压升高患者的年龄偏大、出血量大、GCS评分低、肺部感染发生率高者预后差。结论高血压脑出血患者,血压升高是病情危重的标志之一,血压水平是重要的预后影响因素。
Objective To explore the relationship between the blood pressure level and prognosis of intracerebral hemor- rhage (ICH) patients. Methods One hundred and thirty patients with primary ICH confirmed by brain CT in our hospital were collected. Single factor and multiple linear regression analysis were carried out for natural information, first blood pressure re- cording, the amount of bleeding, Glasgow coma scale (GCS) at the arriving hospital time, the occurrence of lung infection and three months follow-up modified ranking scale (MRS). Results The age, GCS score, intraventricular hemorrhage, diastolic blood pressure, midline shift, lung infections were the main factors of prognosis. The high blood pressure patients had old age, large amount of bleeding, low GCS score, high occurrence rate of lung infections and poor prognosis. Conclusion High blood pressure is one of the critical hallmarks for ICH patients. Blood pressure levels are important prognostic factors.
