上皮性卵巢癌DNA-PKcs、ERCC1表达与耐药相关因素分析 被引量:4

摘要 目的探讨上皮性卵巢癌组织中DNA-PKcs、ERCC1表达及其与临床病理特征及化疗耐药之间的关系。方法采用免疫组化方法检测64例上皮性卵巢癌组织中DNA-PKcs和ERCC1表达情况,并分析其与患者各临床病理参数间的关系。结果 DNA-PKcs、ERCC1阳性表达率在铂耐药患者分别为73.08%、65.38%,在铂敏感患者分别为31.58%、28.95%,铂耐药患者阳性表达率高于铂敏感患者,差异有统计学意义(P<0.05,P<0.01);DNA-PKcs、ERCC1表达与患者发病年龄、FIGO分期、病理类型、病理分级、淋巴结转移、大网膜转移无关(P>0.05),与CA125半衰期、残留灶直径显著相关(P<0.01);DNA-PKcs与ERCC1表达呈正相关(r=0.321,P=0.01);多因素分析显示,DNA-PKcs、ERCC1表达及残留灶直径是卵巢癌患者化疗耐药的独立危险因素。结论 DNA-PKcs、ERCC1表达与化疗耐药相关,联合检测可以预测化疗敏感性。 Objective To explore the expression of DNA-PKcs, ERCC1 and the relationship between it and clinic- pathological characteristics and chemotherapy resistance in epithelial ovarian cancer patients. Methods Expression of DNA- PKcs and RECC1 were detected in 64 epithelial ovarian cancer tissues by immune-histochemical method. Results The posi- tive rate of DNA-PKcs and ERCC1 expression of platinum resistant patients were 73.08% and 65.38% respectively. The posi- tive rate of DNA-PKcs and ERCC1 expression of platinum sensitive patients were 31.58% and 28.95% respectively. The posi-tive rate of platinum resistant patients was higher than platinum-sensitive patients, the difference was significant ( P 〈 0.05, P 〈0. O1 ). No evidence wag found that the expression of DNA-PKc~ and ERCC1 was correlated to the patients'age, FIGO stage, pathological type, pathological grade, lymph node metastasis, omentum majus metastasis ( P 〉 0.05 ), and there was significant correlation with the half-life of serum CA125 and the diameter of residual tumor ( P 〈 O. 01 ) ;The over- expressions between DNA-PKcs and ERCC1 shows positive correlation( r = 0. 321, P = O. 01 ). According to muhifactor anal- ysis, the expressions of DNA-PKes and ERCC1 and the diameter of residual tumor were independent risk factors closely related to chemotherapy resistance in epithelial ovarian cancer patients. Conclusion The expression of DNA-PKcs and ERCC1 was correlated with the chemotherapy resistance in epithelial ovarian cancer patients, the co-expression DNA-PKcs and ERCClin ovarian cancer tissue could be a predictive indictor of platinum resistance susceptibility.
