金属内支架置入在梗阻性左半结肠癌一期切除吻合中的应用研究

Application of metal stent implantation in one-stage management of obstructing left-sided colon cancer

目的:探讨经内镜金属支架置入术在梗阻性左半结肠癌一期切除吻合治疗中的临床应用价值。方法:回顾性分析梗阻性左半结肠癌患者36例临床资料,其中支架治疗组12例,经内镜支架置入后解除梗阻,择期行手术治疗;非支架治疗组24例,经传统术前准备后行手术治疗。比较两组患者一般情况、围手术期情况及并发症发生率的差别。结果:两组在性别、年龄、组织学分类、分化程度及TNM分期等一般情况无显著性差异(P>0.05),两组在手术时间、术中出血、术后禁食天数、术后排气天数、腹腔引流天数、术后住院天数及并发症发生率等方面的差异有统计学意义(P<0.05)。结论:经内镜支架置入术可有效解除梗阻,在梗阻性左半结肠癌一期切除吻合治疗中具有较高的临床应用价值。

Objective： To evaluate the application of endoscopic mental stent implantation in one-stage management of obstructing left- sided colon cancer. Methods： A retrospective study contained 36 cases with obstructing left-sided colon cancer. 12 cases were treated with endoscopic metal stenting as a bridge to an elective resection. The others were treated with surgery without stent. The surgical time, blood loss, postoperative fasting days, postoperative gas passage days, postoperative abdominal drainage days, postoperative hospital stay time and complication rate were compared between the two groups. Results： The two groups were similar in age, sex distribution, histo- logical grade, tumor differentiation and TNM stage. Compared with the contrast group, the stent-group was shorter in surgical time, blood loss, postoperative fasting days, postoperative gas passage days, postoperative abdominal drainage days, postoperative hospital stay time and complication rate. Conclusion： Metal stenting for one-stage management of obstructing left-sided colon cancer could be safely per- formed with successfully.
