重型颅脑损伤患者早期护理干预的效果研究

Effect of Early Nursing Intervention on Patients with Severe Traunmtic Brain Injury

目的探讨重型颅脑损伤患者早期护理干预的效果,为颅脑损伤患者的护理方案提供依据。方法将该院收治的72例重型颅脑损伤患者随机等分为对照组和干预组(n=36),仅干预组给予早期护理干预;分析术后即刻、24h、48h和72h的颅内压变化情况,护理前后的格拉斯哥昏迷量表评分(GCS),治疗后的临床疗效及并发症发生情况。结果观察组术后48h和72h的颅内压低于对照组(P<0.05),护理后的GCS评分高于对照组[(4.35±1.19)vs(3.26±0.81),P<0.01];治疗有效率高于对照组(94.44%vs77.78%,P<0.05),在消化道出血、关节僵硬、继发感染和低氧血症的发生率均低于对照组(P<0.05)。结论重型颅脑损伤患者实施早期护理干预的效果较好,可提高治疗有效率并降低并发症发生率。

Objective To explore the effect of early nursing intervention on patients with severe traunmtic brain injury and provide reference for the nursing protocol. Methods 72 patients with severe traunmtic brain injury admitted by our hospital were randomly divided into Control group and Observation group （n=36）. Only the Observation group received early nursing intervention. The in- tracranial pressure changes of both groups immediately after operation or at 24th, 48th and 72nd h post operation were analyzed. The Glasgow Coma Scale score （GCS） and clinical efficacy before and after the treatment were also investigated. The complications during the treatment were record. Results The intracranial pressures of Observation group at 48th and 72nd h were lower than those of Control group （P〈0.05）. The Observation group＇s GCS score [（11.62＋2.29） vs. （8.35：t：1.85）, P〈0.01] and treatment efficien- cy （94.44% vs. 77.78%, P〈0.05） after nursing were higher than the Control group. The rates of gastrointestinal bleeding, joint stiff- ness, secondary infection and hypoxemia of Observation group were lower than those of the control group （P〈0.05）. Conclusion The effect of early nursing intervention on patients with severe traumatic brain injury was better, improving the treatment efficiency and reducing the incidence of complications.
