明胶海绵片在泪囊鼻腔吻合术中的应用

Application of gelatin sponge sheet in dacryocystorhinostomy.

目的 观察泪囊鼻腔吻合术应用两种不同填塞物的疗效。方法 将 6 0 0例患者随机分成两组 ,分别在手术中采用明胶海绵片 (336眼 )和凡士林纱条 (30 6眼 )做为吻合道的填塞物 ,术后观察成功率 ,炎性反应及出血等情况。结果 应用明胶海绵组手术成功率 97 32 % ,炎性反应 17 2 6 % ,无术后出血。应用凡士林纱条组手术成功率 89 2 2 % ,炎性反应 83 0 1%。结论 泪囊鼻腔吻合术中应用明胶海绵做为吻合道的填塞物 ,它不仅提高了手术的成功率 ,而且明显减少了术后的并发症 ,其疗效明显优于凡士林纱条 (P <0 0 0 5 )。

Objective To observe the clinical effect of applying gelatin sponge sheet (GSS) as the plugging material in dacryocystorhinostomy instead of vaseline gauze(VG).Methods There were 600 patients with chronic dacryocystitis in this prospective randomiaed control study,336 eyes of which were in the experimental group(with GSS),and 306 eyes in the control group (with VG).We observed the success rate,postoperative complications and hemorrhages.Results The success rate in GSS group was 97.32%,only 17.26% cases had mild infection and no hemorrhange.The success rate in VG group was 89.22%,83.01% cases had infection and 73.86% cases had hemorrhanges.Conclusion In order to have a higher success rate and fewer postoperative complications and hemorrhages,We suggest that during the performance of dacryocystorhinostomy,the GSS can be used to replace the VG,which was often used as a plugging material in the clinics.
