摘要
目的通过观察2型糖尿病合并急性脑梗死患者血浆溶血磷脂酸(LPA)、超敏C反应蛋白(hs-CRP)水平,探讨LPA、hs-CRP与2型糖尿病及其合并缺血性脑血管病变之间的关系。方法选择2型糖尿病合并脑梗死患者42例作为DACI组,2型糖尿病且无糖尿病急慢性并发症患者41例作为T2DM组,门诊健康体检者46例作为正常对照组。所有入选者于入院后空腹取血检测血浆LPA、血清hs-CRP、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、TC、TG、LDL-C、HDL-C水平。结果 DACI组LPA、hs-CRP水平高于T2DM组和正常对照组;T2DM组LPA、hs-CRP水平高于正常对照组。DACI组各因素相关分析显示LPA与HbA1c(r=0.352,P<0.01)、hs-CRP(r=0.239,P<0.05)、TG(r=0.319,P<0.01)呈正相关,与FBG、TC、LDL-C及HDL-C无相关关系(P均>0.05);hs-CRP与TG(r=0.387,P<0.01)成正相关,与FBG、HbA1c、TC、LDL-C及HDL-C均无相关关系(P均>0.05)。结论 LPA、hs-CRP测定对预测2型糖尿病患者发生急性缺血性脑血管病变具有重要意义。LPA与HbA1c水平有关,并与体内炎症反应相关。
Objective It is to explore the relationship between LPA, hs - CRP and type 2 diabetes millitus (T2DM) com-plieating with ischemic cerebral vascular disease by observing the levels of plasma LPA and hs - CRP in patients with T2DM and acute cerebral infarction. Methods 42 cases of patients with T2DM and cerebral infarction were chosen as DACI group, 41 cases of patients with T2DM and without acute or chronic complications induced by T2DM were chosen as T2DM group, 46 healthy subjects as normal control group. The levels of plasma LPA, serum hs - CRP, FBG, HbAlc, TC, TG, LDL - C, HDL - C in all subjects were determined after hospitalization. Results The levels of LPA, hs - CRP in DACI group were high- er than that in T2DM group and normal control group, and the levels in T2DM group were higher than that in normal group. Correlated analysis of every factor in DACI group showed that LPA was positive related with HbA1 c (r = 0. 352, P 〈 0. 01 ) , hs - CRP(r =0.239, P〈0.05) , TG(r =0.319,P〈0.01) , and it was not related with FBG, TC, LDL-C and HDL-C(P〉 0. 05 ) ; hs - CRP was positive related with TG9 ( r = 0. 387,P 〈 0.01 ) , and not related with FBG, HbA1 e, TC, LDL - C and HDL- C( P 〉0.05). Conclusion Determination of LPA, hs -CRP has an important significance for predicting acute ische-mic vascular disease in patients with T2DM. LPA is related with the level of HbAle and inflammation actions in vivo.
