慢性阻塞性肺病患者抑郁症状及影响因素

目的 观察慢性阻塞性肺病 (COPD)患者抑郁症状及影响因素。方法 56例作常规肺功能、营养体检指标测定和抑郁症状评分 ,其中 33例作 6分钟行走距离 (6MWD)检测。对1 6例体育锻炼组 (EG)和 40例非体育锻炼组 (NEG)进行 2年随访观察。1 0例进行松静内养功锻炼 4月。结果 COPD组抑郁症状 (DS)比同年龄健康对照组为重。 FEV1 /FVC.6MWD和体重与 DS呈负相关 ,有统计学意义。EG组 2年随访时 DS无明显变化 ,而 NEG组 DS明显加重。松静内养呼吸操锻炼 4个月后相同的运动强度下 ,其呼吸频率减低 ,呼吸困难减轻 ,负荷呼吸时胸腹矛盾呼吸减轻 ,DS改善。结论 COPD患者常伴有抑郁症状 ,与躯体功能有关 。

Objective To study depression and it is influenced facters in chronic.Obstructive pulmonary disease(COPD)patients.Methods 56 cases with COPD were examined their ventilatiry fuctions nutritional and parameters items,depression scores and 6 minutes wolking distance(6MWD)among 33 cases.16 cases tooking part in exercise traning and 40 cases no taling part were followed up for 2years.10 patients took rehabilition programs of Chinese traditional breathing training for 4 months.Results There were statistical difference in scores of depression between patients with COPD and healthy men(P<0.05).Relative analyses showed threr were the linear relationsh between scores of depression and FEV1/FVC or 6 MWD or body weight.There were no changes of depression scores in he patients tooking part exercise for 2 years,but the depression scores of those not taking part one became worse.Breathing exercise training could impreve scores of depression.Conclusion exercise tranings and are the effective methods to improve depression scores for COPD.
