血胱抑素、β_2微球蛋白、超敏C蛋白联合检测2型糖尿病早期肾损害诊断价值

The diagnostic significance of early renal damage in type 2 diabetes detected by the combination of cystatin,β_2-microglobulin and high-sensitivity C-reactive protein

目的:探讨联合检测血清胱抑素(CYS)、血清β2微球(β2MG)、血清超敏C蛋白(CRP)在2型糖尿病(DN)早期肾损害诊断的价值。方法:选取我院诊治的2型糖尿病患者91例,以24h尿清蛋白排泄率(UAER)分为肾功能正常组和轻度肾功能受损组两组,及同期我院体检中心健康体检人员121例为对照组,对各组血清CYS、血清β2微球、血清超敏C蛋白结果进行回顾性调查研究。结果:糖尿病患者肾功能正常组CYS、β2MG、CRP与正常对照组比较均无显著差异;糖尿病患者早期肾功能损害组CYS、β2MG、CRP较正常对照组比较有显著差异;且3项联合检测其阳性率为93.62%特异性为93.18%均高于单项检测。结论:联合检测血清CYS、血清β2MG、血清CRP对2型糖尿病早期肾损害有较高的诊断价值。

Objective： To discuss the diagnostic significance of early renal damage in type 2 diabetic nephropathy （DN） detected by the combination of cystatin, β2-microglobulin （β2MG） and high- sensitivity C-reactive protein （hs-CRP）. Methods： 91 type 2 diabetes patients hospitalized in our hospital were divided into 2 groups, the normal renal function group and the mild renal impairment group based on the UAER with 24 hours. Meanwhile, 121 healthy persons performed physical ex- amination in our Physical Examination Center were selected as the control β2MG, hs-CRP results of the patients in both groups were restrospective gr ly oup. The serum CYS, surveyed and studied. Results： The CYS,β2MG and CRP of the diabetes patients in the normal renal function group were no significant difference from those of the control group; but the CYS,β2MG and CRP of the diabe- tes patients in the mild renal impairment group were obvious different from those in the control group. And the positive rate detected by the combination method was 93.62%, which was higher than the positive rate detected by the single detection. Conclusion： The combination of cystatin, 132MG and hs-CRP in detecting serum CYS,β2MG and CRP will have higher values in diagnosing the early renal damage of type 2 diabetes patients.
