摘要
目的:探讨1区滑车及肌腱损伤的修复方法.方法:自1998年以来,应用显微外科技术修复1区滑车及肌腱损伤66例.用3-0和5-0针线缝合滑车单纯断裂,用肌腱条修补滑车部分缺损.用钢丝钮扣法对合肌腱断端后,用5-0针线Kessler’法缝合肌腱,7-0针线连续缝合断端.结果:49例经过4月~11月随访。根据TAM标准,本组优良率87.76%。讨论:此方法修复的滑车功能良好,对肌腱血远影响小,缝合后肌腱断端牢固,术后可早期功能锻炼,减少了粘连的发生。
Objective: To discuss the repair of finger pulley and flexor tendon injury in zone 1. Methods: Finger pulley and flexor tendon injuries of 46 cases were repaired by microsurgery. Pulley break was sutured with 5-0 nylon and 7-0 nylon. Pulley defect was restored by tendon strip. Flexor tendon was sutured by steel wire and button technique, and then Kessler technique with 5-0 nylon and continuing suture with 7-0 nylon. Results: The excellent and good rare was 87.76% according to TAM standard. Conclusions: Satisfactory result could be achieved after microsurgery repair of pulley and flexor tendon in zone 1. This method showed pulley function satisfactorily, suture tendon firmly, influenced blood circulation of tendon slightly and tamed postoperative digital extension movement early.
