彩色多普勒超声在腹主动脉瘤腔内隔绝术前的应用

A study before abdominal aortic aneurysm endovascular graft exclusion by color Doppler flow imaging

目的 探讨彩色多普勒血流显像 (CDFI)在腹主动脉瘤 (AAA)腔内隔绝术术前的应用价值。方法应用CDFI对 32例AAA患者术前进行检测 ,主要检测内容包括瘤体的大小、瘤体近远端颈的长度、内径及双侧髂总动脉的内径 ;对瘤体、瘤颈及双侧髂总动脉的形态进行观察 ;判断患者行AAA腔内隔绝术的可行性。结果 术前超声诊断的特异性、敏感性均为 10 0 % ,2 3例行手术治疗的患者中 ,术前超声对 2 0例患者术式的选择作出了正确的判断 ,准确率达 86 .9%。结论 CDFI简便、无创 ,在腔内隔绝术前可对AAA进行准确的测量和正确评价 。

Objective To evaluate the diagnostic value of color Doppler flow imaging(CDFI) before endovascular graft exclusion (EVGE) of abdominal aortic aneurysms (AAA).Methods Thirtytwo cases of AAA patients were detected before operation. The contents detected preoperation include:①the size of aneurysm, the length and the distance between proximal and distal neck;② the morphology of aneurysm, proximal neck and biiliac arteries.Results All of the 32 patients were definitely diagnosed by ultrasonography before operation,which specificity and sensitivity were 100%. Of the 23 patients who underwent surgery treatment, ultrasound judgment was correct in 20 patients. The accurate rate was 86.9 %. Conclusions CDFI can accurately measure and correctly assess AAA before operation, and judge the feasibility of EVGE.
