不可触及乳腺病变的术前X线引导下导丝定位 被引量:4

Preoperative Wire Guided Localization Under X-ray Guidance for Nonpalpable Breast Lesions

摘要 目的比较全视野数字乳腺X线摄影引导下导丝定位与体表定位切除不可触及乳腺病灶的准确性。资料与方法回顾性分析采用导丝定位和体表定位患者各50例,比较两组患者术前定位、手术及病理结果,分析两种定位方法的准确性。结果 50例导丝定位患者中成功定位49例,1例导丝移位未切到病灶,1例出现迷走神经反应。50例体表定位患者中病灶切除40例,10例未切到病灶,准确性为80%,无明显并发症。结论全视野数字乳腺X线摄影引导下导丝定位较体表定位准确性高,适用于不可触及乳腺病变的术前定位。 Purpose To compare the accuracy of the wire guided localization under the guidance of full-field digital X-ray mammography with body surface positioning for the excision of nonpalpable breast. Materials and Methods 50 nonpalpable breast disease patients preoperative localization performed with wire guided localization and 50 patients preoperative localization performed with body surface positioning respectively were included, the preoperative localization method, surgical and pathological results were compared to analyze the accuracy of the two methods. Results Among all the patients, wire guided localization were successful in 49 cases, 1 lesion was not excised due to guidewire shifting, 1 case with vagus nerve reaction. Among the 50 patients of body surface positioning, breast lesion was removed in 40 cases, the lesion was uncut in 10 cases, with an accuracy of 80% and no significant complication occurred. Conclusion Wire guided localization under the guidance of full-field digital X-ray mammography has a higher accuracy than body surface positioning, and it is suitable for preoperative localization of the nonpalpable breast lesions.
