宝石能谱CT在结核性胸腔积液和癌性胸腔积液鉴别诊断中的应用 被引量:26

摘要 目的探讨宝石能谱CT在结核性胸腔积液和癌性胸腔积液鉴别诊断中的应用价值。资料与方法经临床确诊的8例结核性胸腔积液和8例癌性胸腔积液患者行宝石能谱扫描,分别测定结核性胸腔积液和癌性胸腔积液的混合能量CT值、能谱曲线斜率、脂基密度值、血基密度值及有效原子序数值,并进行比较。结果结核性胸腔积液和癌性胸腔积液的能谱曲线互相交叉、重叠,无特定的分布规律;结核性胸腔积液和癌性胸腔积液的混合能量CT值、能谱曲线斜率、脂基密度值、血基密度值及有效原子序数比较,差异均无统计学意义(t=―0.891、0.69、0.085、0.581、0.201,P>0.05)。结论宝石能谱CT的能谱成像中混合能量CT值、能谱曲线斜率、脂基密度值、血基密度值及有效原子序数尚不能用于鉴别诊断结核性胸腔积液和癌性胸腔积液。 Purpose To investigate the application value of spectrum CT in the differential diagnosis of tuberculous pleural effusion and malignant pleural effusion. Materials and Methods Spectrum CT was applied to eight patients with tuberculous pleural effusion and eight patients with malignant pleural effusion based on clinical diagnosis. The mixing energy CT value, spectral slope of the curve, fat-density, blood- based density and effective atomic number values were measured and compared about the tuberculous pleural effusion and malignant pleural effusion. Results The spectroscopy curves of tuberculous pleural effusion and malignant pleural effusion was cross and overlapping each other, and there was no stationary distribution. It was demonstrated on significant difference for the mixing energy CT value, spectral slope of the curve, fat- density, blood-based density and effective atomic number values between tuberculous pleural effusion and malignant pleural effusion (t=-0.891, 0.69, 0.085, 0.581 and 0.201, P>0.05). Conclusion The mixing energy CT value, spectral slope of the curve, fat-density, blood-based density and effective atomic number values of spectrum CT is not applied to the differential diagnosis of tuberculous pleural effusion and malignant pleural effusion.
