急性高容量血液稀释对老年患者术后认知功能和脑氧代谢的影响 被引量:8

摘要 目的观察急性高容量血液稀释(AHH)对老年患者术后认知功能和脑氧代谢的影响。方法择期行食道癌根治术老年患者60例,年龄60~83岁,ASAⅠ~Ⅱ级,随机分为急性高容量血液稀释组(AHH组)和对照组(C组),每组30例。AHH组在麻醉诱导后手术开始前以25 ml/min的速率输注6%羟乙基淀粉15 ml/kg,C组不行AHH。连续监测血流动力学指标,并于诱导后AHH前(T0)、AHH后15 min(T1)、AHH后60 min(T2)、术毕前60 min(T3)和术毕时(T4)分别采集桡动脉和右侧颈内静脉球部血样,测定血气值和乳酸值,计算桡动脉-颈内静脉球部血氧含量差(Da-jvO2)、脑氧摄取率(CEO2)和桡动脉-颈内静脉球部乳酸差(VADL)。于术前1 d和术后1、3、7 d用简易智力状态检查表(MMSE)评估患者的认知功能。结果与C组比较,AHH组T1-4时SjvO2升高,Da-jvO2和CEO2降低(P<0.05),VADL变化无显著性差异(P>0.05)。AHH组术后1,3,7 d MMSE评分高于C组(P<0.05),术后认知功能障碍(POCD)诊断人数低于C组(P<0.05)。结论 AHH应用于无心肺疾病的老年患者安全可行,并可减少术后POCD的发生,其机制可能与改善脑氧代谢有关。
