A short-term clinical evaluation of treatment of periodontitis with Er:YAG laser and sonostat VectorTM-system: a split-mouth controlled study

摘要 目的:比较使用Er:YAG激光和Vector超声治疗仪治疗中度慢性牙周炎的短期临床疗效及龈下菌斑中螺旋体的比例变化。方法:单盲法随机分列口实验纳入36例慢性牙周炎患者,分别进行龈下刮治和根面平整术(scaling and root planing,SRP),A组应用Er:YAG激光,B组应用Vector超声治疗仪。在基线和SRP后1、3个月测量探诊深度(PD)、龈沟出血指数(SBI)、附着水平(AL)及龈下菌斑中螺旋体的百分比(S%)。结果:两组在基线时PD、SBI、AL、S%无差异(P>0.05);SRP后1个月和3个月时各项指标与基线相比以及两组间相比均有明显下降(P<0.05)。A组各项指数均低于B组(P<0.05)。结论:应用Er:YAG激光和Vector超声治疗仪进行SRP后均能达到良好的短期临床和微生物效果,Er:YAG激光组的治疗效果优于Vector超声治疗仪组。 AIM: To observe the short-term clinical effects of Er:YAG laser and sonostat VectorTM--system periodontal therapy in the treatment of moderate periodontitis and the change of spirochete proportion in subgingival plaques. METHODS:In this randomized single-blinded split-mouth design study, 36 patients with chronic periodon- titis were treated by scaling and root planing(SRP) and then by Er:YAG laser group (A) and sonostat VectorTM -sys- tem group(B) respectively. The probing depth (PD), sulcus bleeding index (SBI), attachment level (AL) and the proportion of spirochete ( S% ) in subgingival plaques were measured on baseline, 1 and 3 months after treatment respec- tively. RESULTS:One or three months after periodontal basic treatment PD, SBI, AL and S% in both groups de- creased significantly ( P 〈 0.05). Furhermore, group A showed more decrease of the clininal parameters than group B (P 〈 0.05 ). CONCLUSION : SRP with Er: YAG Laser or VectorTM - system both can obtain favorable clinical and microbial effects in the treatment of chronic periodontitis, Er: YAG Laser has better effect than VectorTM system.
