上颌前牙区骨量不足种植修复的美学观察

Esthetic observation for the implant-support prosthesis of insufficient bone in the anterior maxilla zone

目的:评估上颌前牙骨量不足种植修复的美学效果。方法:25例上颌前牙骨量不足的缺牙患者接受种植手术及软硬组织移植术,共植入44颗种植体,修复后追踪1年以上。结果:经1～3年的临床随访,25例患者44颗植体的存留率为100%。13例患者行软组织手术成形的粘膜瓣均成活,术后1年收缩率平均为52%。红色美学指数评分,18例为高得分组,7例为中得分组。10例女性患者出血指数均为0,15例男性患者出血指数为0或1。单个种植体植入,龈乳头Jemt分级为3度,多种植体植入时,Jemt分级为1到3度。冠修复半年后行问卷调查,患者满意度为100%。结论:前牙种植区骨量不足经过软硬组织移植后美学效果基本能达到患者要求,但随访时间稍短,特别对于软组织移植的效果还需做长时间的随访观察。

Objective To evaluate esthetic results for the implant-support prostheses of insufficient bone in the anterior maxilla. Methods 25 cases with insufficient bone in the anterior maxilla were undergone soft tissue and bone reconstruction and implant treatment.A total of 44 implants were placed.Tracking more than one year after the restoration. Results Short-term planting survival rate was 100%.All mucosa flaps of 13 patients underwent soft tissue surgery are survival. Mean shrinkage of mucosa flap jwas52%.18 cases in high group of PES values, 7 cases in middle group of PES values.10 cases of female patient BI are 0, 15 male patients BI are 0 or 1. Gingival papilla Jemt classification for 3 when plant one implant,Jemt classification for 1 to 3 when plant more than one implants. Patient＇s satisfaction rate was 100% after one year. Conclusion After soft tissue transplantation,aesthetic of insufficient bone in the anterior maxilla can achieve patient＇s require.Soft tissue transplantation effect still need long follow -up.
