右眼外伤后继发性青光眼

Glaucoma secondary to ocular trauma in right eye

患者因右眼外伤后8年,视力下降5年,胀痛20d入院。体格检查发现右眼晶状体完全白色混浊,前囊膜下可见结晶样反光物质。眼部B超和眼眶CT发现右眼球壁异物。初步诊断为右眼继发性青光眼、右眼外伤性白内障。继发性青光眼的病因鉴别主要包括眼内铁锈沉着症和晶状体溶解性青光眼。患者入院后行右眼白内障超声乳化吸除术,术后随访,眼压控制良好。讨论体会:外伤后继发性青光眼,需分析具体原因,采取针对性治疗。

The patient, who had ocular trauma 8 years ago, was admitted because of decreased vision for 5 years, and pain for 20 days in right eye. Ocular examination found severe cataract and sub-capsular crystalline material in right eye. The B scanning and CT exam revealed intraocular foreign body in right eye. The initial diagnosis was ＂secondary glaucoma, Traumatic cataract＂. It has to be differentiated that which is the main reason of secondary glaucoma, siderosis or phacolytic glaucoma. Finally, this patient received phacoemulsification in right eye, and the eye pressure was well controlled after the surgery. The exact reason of secondary glaucoma after ocular trauma should be analyzed, and better treatment should be provided. （Chin J Ophthalmol and Otorbinolaryngol,2013 ,13 ：199-202）
