摘要
目的:比较可降解珊瑚羟基磷灰石与脱矿无机牛骨(BIO-OSS)材料在上颌窦提升及种植修复后骨改建情况。方法:选择上颌窦提升种植修复的牙列缺损患者20例并随机分为2组(n=10),A组用可降解珊瑚羟基磷灰石,B组用BIO-OSS为上颌窦提升植骨材料,6月后植入种植体,植入4~6月后进行种植体上部修复。所有患者在5个临床时期进行曲面体层片检查,于中间种植体顶端平面选取定点测量其垂直向骨高度:①中间种植体中点(P1);②与中间种植体远中外缘相距2 mm的位点(P2);③距最远中的种植体的远中外缘5 mm的位点(P3)。统计学比较2组垂直向骨高度的变化。结果:经12~40月的临床追踪观察发现,2组患者的种植体留存以及每组内部P1、P2与P3之间垂直向骨高度改变无统计学差异;A组骨组织替代材料在机体内改建高度变化值大于B组。结论:在上颌窦提升种植修复过程中,可降解珊瑚羟基磷灰石骨替代材料在植入机体后其改建速度快于脱矿无机牛骨。
Objective: To compare the graft height between coral hydroxyapatite and dimenerialized bovine bone (BIO-OSS) after transplantation for maxillary sinus floor elevation(SFE). Methods: 20 patients underwent SFE were randomly divided into 2 group. Coral hydroxyapatite was grafted in group A, and BIO-OSS in group B. 6 months after transplantation dental implant was placed, and 4 - 6 months after implantation prosthesis was completed. The graft height was measured on panoramic radiographs during follow- up at the level of the middle implant crest module : ① to the middle of the implant ( P1 ), ② to the site of 2 mm distal of the implant middle (P2) and ③ to the site of 5 mm distal to the implant middle(P3). Data were statistically analysed. Results: After 12 -40 month fol- low up, All implants(38 in group A and 40 in group B) were survived with satisfactory function. Vertical height change at each site in each group was not statistically different. The graft height change in group A was more than in group B ( P 〈 0.05 ). Conclusion: The coral hydroxyapatite may be reconstructed more quickly than BIO-OSS following transplantation for maxillary sinus floor elevation.
