胰岛素抵抗与脑梗死危险因素关系的研究

The study of relationship between insulin resistance and risk factor of cerebral infarction

目的 :研究胰岛素抵抗与动脉硬化性脑梗死危险因素的关系。方法 :用常规方法测定 91例脑梗死患者和 80例健康人的空腹血糖、胰岛素、血脂、组织型纤溶酶原激活物 (t-PA)、组织型纤溶酶原激活物抑制物 (PAI-1 )和血压水平。结果 :脑梗死组的胰岛素敏感性指数 (ISI)显著低于正常对照组 ,ISI与收缩压、舒张压、TG、APOB、PAI-1水平负相关 ,与 HDL正相关。结论 :胰岛素抵抗与高血压、高 TG、低 HDL血症和高 PAI-1活性等脑梗死危险因素形成密切相关。因此 ,在动脉硬化性脑梗死防治中 。

Objective:To study the relationship between insulin resistance and risk factor of atherothrombotic brain infarction.Methods:The serum concentration of fasting glulose,insulin,lipids ,the activities of tissuetype plasminogen activator(tPA),plasminogen activator inhibitor1(PAI1)and the level of blood pressure were measured in 91 patients and 80 heathy control subjects.Results:The insulin sensitivity index in the patients with cerebral infarction was significantly lower than the healthy control subjects(P<0.01).ISI in patients with cerebral infarction was negatively associated with the levels of systolic blood pressure(SBP),diastolic blood pressure(DBP)、TG、APOB and PAI1 and positively linked to the concentrtion of HDL.Conclusion:ISI in the patients with cerebral infarction may contribut to the formation of hypertention,hypertriglyceridemia,the decreased concentration of HDL and the increased activity of PAI1.Therefore,improvement of ISI in the patients with cerebral infarction is important in the prevention on and treatment of cerebral infarction.
