泮托拉唑、奥美拉唑对高血压脑出血并上消化道出血疗效分析

Clinical Analysis of Pantoprazole and Omeprazole in the Treatment of Hypertensive Intracerebral Hemorrhage with Upper Gastrointestinal Bleeding

目的评价泮托拉唑与奥美拉唑对高血压脑出血并发上消化道出血的临床疗效,探讨其临床适用性。方法选择2009年5月～2012年11月于我院住院治疗的高血压脑出血并发上消化道出血患者43例,随机分为试验组23例和对照组20例,两组患者均给予相同的对症处理,其中对照组奥美拉唑静脉滴注,试验组则采用静脉点滴泮托拉唑。观察两组患者的治疗有效率及不良反应情况。结果两组患者经治疗后症状均有所缓解,止血效果较佳。试验组患者的显效率达60.87%,有效率达91.03%,而对照组患者的显效率为55.00%,有效率为85.00%,两组患者有效率比较,差异无明显统计学意义(χ2=0.41,P>0.05);两组患者显效率比较,差异也无明显统计学意义(χ2=0.15,P>0.05)。两组患者的不良反应情况比较,差异也无明显统计学意义(P>0.05)。结论泮托拉唑与奥美拉唑对高血压脑出血并发上消化道出血治疗均有效,且安全性可靠,但泮托拉唑的药代动力学特点较为稳定,临床上两种药物根据患者病情需要可以选择应用。

Objective To analyze pantoprazole and omeprazole in the treatment of hypertensive intrac- erebral hemorrhage with upper gastrointestinal bleeding and explore its clinical applicability.Methods 43 patients with hypertensive cerebral hemorrhage ing, in our hospital from May 2009 to November 2012, complicated with upper gastrointestinal bleed- were randomly divided into experimental group of 23 cases and a control group of 20 cases. All patients were given the same symptomatic treatment, in which the control group using omeprazole intravenous infusion, the test group using intravenous pantoprazole.Results Symptoms eased in all patientsafter treatment. The experimental group were markedly effective rate of 60.87% ,the effective rate of 91.03% ,while the control group of patients with markedly effective rate was 55.00% ,the effective rate of 85.00% ,while the difference was not statistically signifi- cant（x/=0.41 ,P〉0.05）;Comparison of two groups of patients with significant efficiency, difference is not statistically significant （x2=0.15,P〉0.05）.Conclusions Pantoprazole and omeprazole in the treatment of hypertensive intracerebral hemorrhage with upper gastrointestinal bleeding were effective and reliable security. However, the pharmacokinetic characteristics of pantoprazole is more stable, two drugs in clinical application be chosen based on the patient＇s condition requires.
