Effect of CagA^+ Helicobacter pylori eradication on the levels of serum hsCRP and matrix metalloproteinase-9 in patients with gastric ulcer

摘要 目的:探讨细胞毒素相关基因A+(CagA+)幽门螺杆菌(Hp)根除前后对胃溃疡患者血清基质金属蛋白酶-9(MMP-9)及高敏CRP(hsCRP)的影响。方法:55例Hp+CagA+胃溃疡患者为根除治疗组,同期就诊的55例Hp-CagA-胃溃疡患者为对照组,根除治疗组患者研究开始时根除Hp治疗采用三联疗法,包括阿莫西林(1 g,2次/日)、克拉霉素(500 mg,2次/日)及埃索美拉唑(20 mg,1次/日),疗程7 d。随访3个月,研究前后同时检测两组患者血清hsCRP及MMP-9。结果:研究开始时,根除治疗组血清胆固醇、hsCRP及MMP-9水平比对照组高(P<0.05);研究结束时,两组血清胆固醇、hsCRP及MMP-9水平比较差异无统计学意义(P>0.05);与研究前比较,根除治疗组研究结束时血清胆固醇、hsCRP及MMP-9水平明显降低(P<0.05)。结论:通过根除Hp+CagA+,可降低胃溃疡患者血清hsCRP及MMP-9水平,降低患者的炎症损伤,因此可促进溃疡愈合。 Objective: To investigate the effect of cytotoxin associated gene A + (CagA + ) Helicobacter pylori (Hp) eradication on the levels of serum high sensitive C reactive protein (hsCRP) and matrix metalloproteinase-9 in patients with gastric ulcer. Methods: A total of 55 patients with gastric ulcer and CagA + Hp (eradication group) and 55 patients with gastric ulcer and without Hp (control group) were recruited in this study. At the be- ginning of study, the patients in eradication therapy group were adminstrated with eradication therapy for a week to Hp. However, the patients in control group were adminstrated with placebo for a week on the basis of general treat- ment. The levels of serum hsCRP and matrix metalloproteinase-9 (MMP-9) were performed at the beginning and the end of study in all subjects. The follow-up period was 3 months. Results: In comparison with control group, the levels of serum total cholesterol, hsCRP and MMP-9 were significantly increased in erdication group at enroll- ment. However there were no significant differences between the two groups in these parameters at the end of this study.
