糖尿病患者胰岛素笔芯与注射笔不配套使用的原因分析与对策

Cause analysis and countermeasures of insulin refill not match insulin pen for diabetes patients

目的分析胰岛素笔芯与注射笔不配套使用的主要原因,并提出相应的对策。方法选择2011年6-12月来我院内分泌科门诊复诊使用胰岛素笔芯治疗的1260例糖尿病患者,统计其胰岛素笔芯与胰岛素注射笔不配套使用的例数,对其胰岛素笔芯与注射笔不配套使用情况进行调查分析,针对其主要原因,提出整改措施。整改措施实施后3个月,再次连续收集6个月门诊糖尿病患者胰岛素笔芯与胰岛素注射笔不配套使用的例数。干预措施前后两组胰岛素笔芯与注射笔不配套使用率比较采用χ2检验。结果干预前胰岛素笔芯与注射笔不配套使用28例,主要原因为患者擅自更换胰岛素注射工具、患者自购不配套胰岛素注射笔、患者错误述说自备笔名称致医生开医嘱错误、宣教方法及工作流程欠完善。干预后胰岛素笔芯与注射笔不配套使用1例,两组间存在显著差异(χ2=28.061,P<0.05)。结论通过改进工作流程及配套使用胰岛素注射笔的健康教育方法后,有效减少了胰岛素笔芯与注射笔不配套使用率,提高了患者使用胰岛素笔芯的安全性。

Objective To study the main cause of insulin refill not match insulin pen for diabetes patients,and put for- ward corresponding countermeasures. Methods The data of 1260 cases of diabetes patients that using insulin pen and admitted in the outpatient department of endocrinology of Taizhou Hospital of Zhejiang Province from June 2011 to December 2011 were analyzed, recorded the cases that insulin refill not match insulin pen,put forward the rectification measures for the main reason.After the rectification of 3 months, continuous collected the cases that insulin refill not match insulin pen for 6 months.Used the Chi-square test to compare the not match usage rate between before inter- vention and after intervention. Results There were 28 cases of not match usage before intervention,the main reasons were patients changed the insulin injection tools without authorization, bought the not matched insulin pen without au- thorization, bought the not matched insulin pen because of telling the wrong name to the doctors,and the inadequate education method and working process;there was only 1 case of not match usage after intervention, there was signifi- cant differences between the two groups （x2=28.061, P 〈 0.05）. Conclusion By improving the work process and pro- ceeding the health education of match usage of insulin refill and insulin pen, the not match usage rate are decreased, and the safety are increased.
