运动加饮食干预对2型糖尿病肥胖患者胰岛素抵抗及血清脂联素水平的影响 被引量:2

摘要 目的:探讨运动加饮食干预对肥胖的2型糖尿病(T2DM)患者胰岛素抵抗(IR)及血清脂联素(ADP)水平的影响。方法:选取肥胖(BMI≥28kg/m2)患者106例,按美国糖尿病学会(ADA)2009年2型糖尿病诊断标准将患者分为糖尿病组(DM组)46例和非糖尿病组(即糖耐量正常、非DM组)60例。测定空腹血糖(FBG)、空腹胰岛素(FINS)、血脂[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、ADP,计算胰岛素抵抗指数(HOMA-IR)及体质指数(BMI)。予DM组为期8周运动干预及饮食控制治疗,8周后再次检测上述指标。结果:(1)干预前DM组FBG、FINS、HOMA-IR、TG、LDL-C水平显著高于非DM组,ADP水平显著低于非DM组,差异均有统计学意义(P<0.05)。Spearman线性相关分析显示,HOMA-IR与年龄、TG呈正相关(均P<0.05),与ADP水平呈负相关(P<0.05)。(2)干预后DM组BMI、FBG、HOMA-IR高于非DM组,而ADP水平低于非DM组,差异均有统计学意义(P<0.05);且DM组FINS、TG、LDL-C、BMI水平较干预前下降,差异有统计学意义(P<0.05);Spearman线性相关分析表明:ADP水平与肥胖、FBG水平呈负相关(均P<0.05)。结论:肥胖的T2DM患者存在低ADP及高HOMA-IR,运动及饮食干预能有效降低T2DM患者血脂、ADP水平,改善胰岛素抵抗。
