神经外科工作站在内镜经鼻蝶垂体瘤手术中的应用 被引量:6

摘要 目的:探讨神经外科工作站对内镜经鼻蝶入路垂体瘤手术的指导作用。方法 :自2009年9月至2012年11月我科行内镜经鼻蝶入路垂体瘤切除术70例,术前均采用64排CT扫描及工作站进行三维重建。比较术前工作站三维重建图像和术中内镜图像并行经鼻蝶入路中解剖结构累加分值Spearman秩相关分析;利用工作站辅助定位重要解剖标志,精确量化三维解剖关系。结果:术前工作站三维重建图像与术中内镜图像基本一致,相符性好,10个解剖标志显示的累加分值具有显著相关关系(r=0.864,Р<0.001);工作站重建虚拟图像可模拟内镜经鼻蝶入路垂体瘤手术,准确辅助定位、观察解剖标志及变异,精确量化个体化的三维解剖关系,设计最佳的鞍底开窗,同时准确显示与手术设计相关的距离参数。结论:神经外科工作站是一种简便实用的辅助性工具,可用于指导内镜经鼻蝶垂体瘤手术术前计划形成,增加手术操作的安全性,提高手术效率。
