颅内动脉瘤显微手术和介入治疗效果对比研究 被引量:8

摘要 目的比较颅内动脉瘤的显微手术和介入治疗的应用价值。方法将197例颅内动脉瘤患者随机分为观察组和对照组,观察组患者选择显微手术,对照组患者选择介入手术,比较两组患者治疗效果、平均住院时间及治疗费用。比较两组患者动脉瘤完全夹闭(闭塞)率及并发症发生情况。结果①两组间痊愈率、好转率、植物状态率及病死率间均无统计学差异(P>0.05)。②对照组患者平均住院时间明显短于观察组(P<0.05)。观察组患者平均治疗费用显著低于对照组(P<0.05)。③观察组患者完全夹闭(闭塞)率显著高于对照组(P<0.05)。④观察组及对照组患者术后并发症发生率比较,差异无统计学意义(P>0.05)。结论与介入治疗相比,颅内动脉瘤的显微手术完全夹闭(闭塞)率高,治疗费用低,但住院时间长。 Objective To compare the value of microsurgery and interventional in the treatment of intracranial aneurysm.Methods 197 patients with intracranial aneurysm patients were randomly divided into observation group and the control group.The patients of the observation group patients were performed microsurgery.The patients of control group were performed surgical intervention.The outcomes,average length of stay and cost of treatment of the two groups patients were compared.The aneurysm complete occlusion(occlusion) rate and occurrence of complications were also compared.Results ①There were no statistically different in the cure rate,improvement rate,vegetative state rate and mortality of the two groups(P 0.05).②The average length of stay for patients of the control group(11.8 ± 3.1) d,was significantly shorter than that of the observation group(P 0.05).The average cost of treatment of the observation group patients(29 876.8 ± 4 128.6) was significantly lower than that of the control group(P 0.05).③Observation group patients totally clipping rate of 91.91%.Total occlusion rate was 71.43% in the control group patients.The observation group patients complete occlusion rate was significantly higher than that of the control group.The difference was statistically significant(P 0.05).The post-operative complications incidence of observation group and the control group patients was 8.08%(8 /99) and 9.18%(9 /98),respectively.The difference of complication incidence of the two was not statistically significant(P 0.05).Conclusion Compared with interventional therapy,the completely occlusion(occlusion) rate,cost of microsurgery therapy of intracranial aneurysms is low,but accompanied with long hospital stay.
