64层螺旋CT冠状动脉造影的临床价值和成像影响因素

Clinical valuation and influence factors on image quality of 64-slice spiral CT coronary angiography

目的:探讨64层螺旋CT在冠状动脉病变中的诊断价值及其成像的影响因素。方法:采用64层螺旋CT对77例可疑冠心病及支架置入术后患者进行冠状动脉成像,通过回顾性心电门控增强扫描并进行最大密度投影重建(MIP)、曲面重建(CPR)、容积重建(VR),评价其对冠状动脉的显示能力、病变的检出率,总结影响成像的各种因素。结果:64层螺旋CT对冠状动脉各分支显示效果良好,有很多因素会影响成像质量。结论:64层螺旋CT在诊断冠状动脉病变方面具有重要价值。影响成像质量的因素主要有病人的心理因素、基础心率及心率的波动、呼吸运动、扫描参数的设定及重建时相的正确选取、管壁严重钙化等。

Objective： To evaluate the value of clinical application of coronary angiography and the influencing factors on image quality with 64-slice spiral CT. Methods： 64-slice spiral CT angiography was performed in 77 patients of suspected coronary heart disease （CHD） and stenting. All patients were examined in ordinary scanning with the technique of retrospective electrocardiography gating, and maximum intensity projection, curved planar reconstruction and volume reconstruction technology were done. The showing rate of coronary artery, the deteeting rate of CHD and the influencing factors on image quality were evaluated by CT. Results： 64-slice spiral CT could show every branch of coronary artery clearly. Many factors affected image quality. Condusion： 64-slice spiral CT has significant value in diagnosing coronary artery disease. The image quality is affected by psychological factors , heart rate and heart rate fluctuation, breath movement, scanning parameter settings, good reconstruction algorithms choieing.
