摘要
目的:解决颈部、腋窝烧伤所致皮肤损害及疤痕挛缩畸形患者臂丛神经穿刺点的定位困难,提高臂丛神经定位的准确性及其阻滞效果。方法:用北京产电子脉冲针灸治疗仪制成经皮神经刺激器,将刺激器输出电极的正极置于手术的上肢某一部位(根据上肢受伤的部位选择,如前臂伤,电极可以置于手部),其负极置于手术侧臂丛神经刺激点的附近,通过观察刺激器的电子脉冲刺激引起上肢的肌肉收缩,确定臂丛神经的位置,再注入局麻药物,而后,观察、记录麻醉效果。结果:实验组与对照组比较,臂丛阻滞效果的优良率提高10.75%。结论:该刺激器为颈部烧伤所致皮肤损害及疤痕挛缩畸形患者臂丛神经阻滞穿刺点的定位提供了客观指标,对提高该患者臂丛阻滞效果有显著意义。
Objective To refit the electron-pulse instrument into a percutaneous nerve stimulator to solve the location problems of piercing the braehial plexus nerves of the patients who have neck or armpit burns which result in the deformation of skin or scar contracture so that brachial plexus location and its block effect can be greatly improved, Methods Using the percutaneous nerve stimulator refitted from a Beijing-made electron-pulse acupuncture instrument, the outlet positive electrode was put onto some point of upper limb, and the negative electrode was set near the stimulating point of the brachial plexus nerve at operating side. The point of the positive electrode was determined according to the burned place, for instance, if the antebrachium was burned, the electrode could be placed on the hand. Then the muscle contraction stimulated by the electronic pulse was studied to locate the brachial plexus nerve. The local anesthetic was administered, the effect of which was observed and recorded.Results The rate of efficiency was increased by 10.75% for brachial plexus block.Conclusion The stimulator determines an objective norm for the piercing location of brachial plexus block of the patients whose neckburn makes the skin deformed or contracted, so it is highly effective in improving the brachial plexus block of the patients.
