Gamma钉与DHS钉治疗股骨粗隆间骨折的临床对照研究

A prospective randomized controlled study of femoral intertrochanteric fracture with gamma nail versus dynamic hip screw

目的比较Gamma钉与DHS钉治疗股骨粗隆间骨折的临床疗效。方法选择2008年1月至2012年5月收治的100例股骨粗隆间骨折患者,将其随机分为两组,在闭合复位的基础上,分别予以Gamma钉与DHS钉固定,观察并比较两组患者手术期各项指标、术后并发症以及术后髋关节功能情况。结果 Gamma钉组的术中失血量及术后并发症的发生率明显低于DHS组,而Kuderna髋关节功能评分优良率要高于DHS组,差异有统计学意义(均P<0.05)。结论 Gamma钉在治疗上较DHS具有操作简单、手术损伤小、骨折固定牢固、疗效可靠、并发症少等优点,适于临床推广。

Objective To compare the clinical efficacy of gamma nail or dynamic hip screw （DHS） in treatment ot temoral intertrochantenc fracture. Methods 100 patients with femoral intertrochanteric fracture who were treated in our hospital were randomly divided into two groups from January 2008 to May 2010. Two groups were respectively given the therapy of gamma nail or DHS after pedicle screw closed reduction.The index dur- ing the operation, postoperative complications and postoperative hip joint function between two groups were observed and compared. Results The intraoperative blood loss volume and incidence of postoperative complications gamma nail group were lower than that in DHS group. Whereas the ex- cellent and good rate of hip joint function scores in gamma nail group were higher than those in DHS group. There were significant differences be- tween two groups （P〈0.05）. Conclusion The treatment for femoral intertrochanteric fracture with Gamma nail has the advantages of simple opera- tion, minimal invasion, confirmed immobilization, confirmed effectiveness and less complications. The procedure is recommended in treatment for femoral intertrochanteric fracture.
