脑室-腹腔分流术治疗336例脑积水的临床分析 被引量:1

摘要 目的探讨脑积水的临床诊断及其行脑室.腹腔分流术的适应证、时机选择、分流管选择和并发症的防治。方法回顾性分析2002年10月至2012年10月间行脑室腹腔分流术的336例患者的临床资料。结果经脑室腹腔分流术治疗的336例患者,其中外伤性脑积水96例,非外伤脑积水240例,随访6个月至3年,根据术后临床症状改善情况评定疗效,外伤性脑积水(随访40例)显效率低于非外伤性脑积水(随访125例)患者(25.0%vs48.0%,X^2=6.56,P〈0.05)。根据术中分流管的选择,可调压分流管术后引起的并发症最少(1.9%)(P〈0.01)。结论对脑积水患者进行正确的诊断、完善的术前检查,严格把握分流指征,正确选择分流时机及分流管,仔细手术操作,对减少术后并发症、改善预后有及其重要的影响。
