国产重组链激酶溶栓对急性心肌梗死病人心功能储备的影响

The effect of thrombolytic therapy with domestic r-SK on the storage of heart function in acute myocardial infarction

目的 探讨国产重组链激酶 (r SK)溶栓治疗急性心肌梗死 (AMI)对于心功能保护及生活质量改善的意义。方法 选择接受r SK溶栓治疗及未溶栓的AMI患者各 3 0例 ,其中溶栓组获得梗死相关血管再通的 19例 ,未再通的 11例 ,并于梗死后 6个月随访时行M 型超声左室功能测定 (LVEF)及运动心肺功能评定。结果 再通组与未通组、未溶栓组比较 ,左室射血分数明显增高 (P <0 .0 1)、运动时间和最大运动负荷量明显提高 (P <0 .0 5 )、运动中最大心率和心率增值显著提高 (P <0 .0 2 )、运动诱发的心电图S T段压低显著改善 (P <0 .0 1)、最大代谢当量 (METS)、每分钟通气量 (VE)和最大氧耗量(Vo2max)均显著增高。结论 r SK静脉溶栓后梗死相关冠脉再通可明显改善心功能储备 ,提高患者的生活质量 ,未通组与未溶栓组比较差异无显著性。

Objective To investigate the role of thrombolytic therapy with domestic r SK in AMI in safeguard of heart function and impromement of life quality. Methods To select 30 patients with AMI who were treated by thrombolytic therapy and other 30 patients who were not.In the thrombolytic group,19 had their infarct relating arteries reopened,11 had not.It was performed to measure of left ventricular with M ultrasound and evulation of cardiopulmonary function during visiting the patients 6 months after the infarction. Results Reopening group was compared with non reopening group,non thrombolyte group:LVEF、the total exercise time and maximal work load were markedly in creased(P<0.05),the maximal heart rate and the increasing value of heart rate were increased(P<0.02),the ST segment depression were induced by exercise(P<0.01),the maximal metabolic equivalents、minute ventilation and maximal oxygen consumption were significantly increased. Conclusion After carrying out thrombolytic therapy with domestic r SK.Reopening of the infarct relating arteries could improve storage of heart function and raise the patients life quality.
