丙型肝炎患者外周血T细胞CD28、CD152表达的检测及其意义 被引量:1

Expressions of CD28 and CD152 on surface of peripheral blood T cells in patients with hepatitis C

摘要 目的:检测丙型肝炎(丙肝)患者外周血T淋巴细胞表面CD28和CD152的表达情况,评价患者的细胞免疫状态。方法:用荧光抗体CD4-PECY5、CD8-PE标记淋巴细胞,并用流式细胞仪分别测定患者CD4+淋巴细胞和CD8+淋巴细胞表面CD28、CD152表达的百分率,并观察其与丙肝病毒(HCV)-RNA、丙氨酸氨基转移酶(ALT)水平间的关系。结果:①与正常对照组比较,慢性丙肝患者CD4+CD28+、CD8+CD28+细胞百分率显著降低,差异有统计学意义(t=2.86,P<0.01;t=2.125,P<0.05);CD4+CD152+细胞百分率显著升高,差异亦有统计学意义(t=3.073,P<0.01);②与ALT正常组比较,ALT异常组表达CD4+CD28+(t=6.361,P<0.001)、CD8+CD28+(t=2.309,P<0.05)、CD4+CD152+(t=2.387,P<0.05)细胞百分率均显著升高,而CD8+CD152+细胞百分率显著降低(t=2.591,P<0.05);③丙肝患者RNA阳性组(≥500拷贝/mL)与阴性(<500拷贝/mL)组间CD4+和CD8+淋巴细胞表面CD28和CD152的表达差异均无统计学意义(P>0.05)。结论:CD28、CD152可能在慢性丙肝免疫损伤中起重要作用,但与血中病毒复制情况及含量无关。 Objective To study the expressions of CD28 and CD152 on surface of peripheral blood T cells in patients with hepatitis C and to evaluate the cellular immunity of the patients. Methods Lymphoeytes were labeled with fluorescence antibody CD4-PECY5, HLA-DR-FITC. The percentage of expression of CD28 and CD152 on T cells was detected by flow cytometry, and its relationship with levels of HCV-RNA and ALT was analyzed. Results ①The levels of CD4+CD28+ and CD8+CD28+ cell were significantly lower (t=2.86, P〈0.01 ;t=2.125, P〈0.05)and the level of CD4+ CD152+ cell was significantly higher (t=3.073, P〈0.01)in hepatitis C patients than in nolnmal controls. ②The levels of CD4+CD28+(t-6.361,P〈0.001 ), CD8+ CD28+(t=2.309,P〈0.05)and CD4+CD152+(t=2.387,P〈0.05)cell were significantly higher and the level of CD8+CD152 cell was significantly lower (t=2.591,P〈0.05)in patients with elevated ALT level than that in patients with normal ALT. ③No significant difference was found in HLA-DR expression on CD4 lymphocytes and CD8 expression on CD28 lymphocytes between patients with abnormal and normal alanine aminotransferase (ALT) level and patients with detectable HCV-RNA (〉t500 copy/mL) copies and undetectable HCV-RNA (〈500 copy/mL) copies (P〉0.05). Conclusions CD28 and CD152 may play an important role in hepatitis C immune damages, but are not correlated with viral replication.
