先天性心脏病复杂畸形MRI与B超及X线血管造影的影像学对比研究 被引量:12

摘要 目的 评估磁共振成像 (MRI)对先天性心脏病复杂畸形的显示能力。材料与方法  2 1例先天性心脏病复杂畸形患者行磁共振平扫、电影检查 ,其中 8例接受动态增强磁共振血管造影检查 (DCE MRA) ,影像资料与B超、X线血管造影检查 (CAG)对比 ,部分结果进行统计学处理 ,15例经手术证实者对照手术所见。结果 共发现心脏及大血管畸形 5 5处 ,磁共振成像、B超和CAG均能很好显示心脏、心脏与大血管连接部分畸形 ;心脏B超对瓣膜病变敏感性高 ;MR、CAG对大血管病变具有优势。DCE MRA能动态、全面显示大血管病变 ,与CAG具有很好的一致性。结论 磁共振心脏大血管检查图像清晰、信息丰富 ,适合于先天性心脏病复杂畸形多发病变的显示 。 Objective To evaluate MRI in displaying complex congenital cardiac diseases.Materials and Methods Twenty one patients (7 females and 14 males, age 3~40) underwent MRI and cine MR. MR angiography was performed in 8 cases. MRI findings were compared with echocardiographic and X ray angiographic findings. The data were analyzed statistically. Fifteen cases underwent surgery, and the surgical findings were compared with the imaging findings.Results A total of 55 abnormalities were found. All MRI, echocardiography and X ray angiography could well show the abnormalities of the heart and great vessels. Echocardiography was very sensitive to valvar disease, though MR and X ray angiography were superior to echocardiography in demonstrating abnormalities of great vessels. With 3 D reconstruction and dynamic display, DCE MRA could show the abnormalities of the heart and vessels in detail, quite consistent with X ray angiography.Conclusion MR imaging is a valuable technique for demonstrating complex congenital heart diseases.
