膝关节外伤性骨髓水肿的MR脂肪抑制术成像 被引量:39

摘要 目的 分析膝关节外伤所致周围骨质骨髓水肿的MRI表现 ,探讨其形成的病理学机制。材料与方法 MRI检查发现膝关节外伤性骨髓水肿 43例 ,除常规T1、T2 加权像检查外 ,均有矢状面和冠状面脂肪抑制术检查。结果  43例患者中共发现膝关节周围水肿 68处 ,常规T1、T2 加权像能显示大部分病灶 ,均呈相对低信号 ,脂肪抑制术后均可显示 ,呈明显的高信号。结论 MRI脂肪抑制T2 加权像能准确显示膝关节周围骨质的外伤性骨髓水肿 。 Objective To analyze MRI features of traumatic marrow edema in bones around the knee and to clarify the mechanism causing edema.Materials and Methods Forty three cases of traumatic marrow edema of the bones around the knee were found on MRI exam. Except routine SE T 1WI and turbo SE T 2WI, fat suppression sequence with sagittal and coronal scanning was performed.Results Sixty eight bone marrow edema were found in 43 cases. Most lesions were in the femur, nest in the tibia. T 1WI and T 2WI demonstrated most lesions with lower signal. With fat suppression, all lesions displayed more clearly, and manifested as higher signal.Conclusion Using fat suppression sequence, T 2WI can precisely demonstrate traumatic bone edema. Probably, the traumatic bone edema is a physiologic response of bone marrow to the trauma, rather than to the fracture.
