摘要
目的:研究毒蛇咬伤血球心肌酶变化及其临床意义。方法:对120例毒蛇咬伤的病人按毒蛇类型分为神经毒、血循毒、混合毒3组,进行血清心肌酶的检测,观察其变化,同时按临床症状轻型、重型、危重型进行对比。结果:120例中心肌酶升高者87例(72.5%)。在心肌酶升高的蛇咬伤中,血循毒和混合毒的CK、CK-MB与神经毒比较有显著差异性(P<0.05),而血循毒与混合毒之间无明显差别(P>0.05),且随着病情加重心肌酶升高的比例增加,十分似急性心肌梗死的改变但通常没有相应临床症状和典型心电图改变。经过及时治疗心肌酶能在短时间内恢复正常。结论:毒蛇咬伤能引起心肌酶谱的改变,尤其是血循毒和混合毒蛇咬伤更为明显,且病情越重心肌损伤的机率越高。临床上及早应用抗蛇毒血清和营养心肌综合治疗能预防心肌受损或使受损心肌完全恢复。
Objective: To investigate the changes of serum myocardial enzyme and its clinical significance in patients with venomous snake bite. Methods: According to the type of snake,120 patients with venomous snakebite were divided into three groups: nerve poison, blood circulation poison and admixture poison.At the same time, according to the clinical manifestation, these patients were divided into mild type,severe type and imminent type.All cases were measured their serum myocardial enzyme level.The changes of serum myocardial enzyme were observed and its clinical significance was analyzed.Results:87 cases serum myocardial enzyme level had gone up in 120 patients,the level of CK, CK-MB in blood circulation poison or admixture poison were remarkable difference with nerve poison (P<0.05), but there were no significant difference between blood circulation poison and admixture poison (P>0.05). The changes of serum myocardial enzyme were alike acute myocardial infarction in some cases. However, there were no correspond clinical manifestation and typical electrocardiogram.Conclusions:The results of this study indicates that venomous snake bite can cause changes of serum myocardial enzyme level.especially blood cireulation poison and admixture poison. the more seriously patient's condition, The higher injury rate of myocardial.Patients with venomous snake bite can be prevented from myocardial injury or complete recovered bymeans of antivenom serum treatment in time and myocardial nutrition support.
