摘要
目的探讨不同护理管理模式在晨间首台择期手术患者中的管理效果及影响。方法选择2012年1—10月进行择期手术的患者240例,随机分为对照组和观察组各120例。对照组按传统接手术患者流程提前30~40 min接患者到手术室,观察组按改良后的护理模式,将患者接入手术室后在麻醉复苏室进行手术患者无缝隙护理模式,由两名护士集中护理管理,并采取相应的人文关怀等护理干预措施。计数资料采用χ2检验,计量资料采用t检验,P<0.05为差异有统计学意义。结果两组术晨30~40 min在手术室等候手术期间,观察组中国版状态焦虑量表(S-CAI)评分[(14.05±2.32)分]明显低于对照组[(16.75±2.33)分],两组比较差异有统计学意义(t=9.00,P<0.05)。在手术室等候手术期间观察组收缩压、舒张压、心率[(17.6±2.5)、(11.6±1.1)mm Hg(1 mm Hg=0.133 kPa)、(88.7±11.6)次/min]低于对照组[(19.8±4.1)、(14.2±2.1)mm Hg、(110.2±8.7)次/min],两组比较差异均有统计学意义(t=5.02、12.01、16.24,均P<0.05)。观察组满意度高于对照组,两组比较差异有统计学意义(χ2=37.42,P<0.05)。结论施行手术患者无缝隙护理模式,术期患者集中护理管理,并实施相应的人文护理干预,可降低患者的情绪反应,减轻其紧张焦虑恐惧心理,增强安全感,提高患者对手术室护理工作的满意度。
Objective To investigate the effect and influence of different nursing management models on patients undergoing the first selective surgery in the morning.Methods From January to October in 2012,240 patients undergoing selective surgery were selected and randomly divided into control group and observation group,120 patients for each.Patients in the control group were sent to operating room 30-40 minutes earlier as usual.Patients in the observation group received improved nursing model in which they were sent to anesthesia recovery room for seamless care managed by two nurses,and meanwhile,nursing interventions such as humanistic care were given.χ2 test was used for count data and t-test for measurement data,the result of P〈0.05 was considered statistically significant.Results During the time of 30~40 min waiting in operating room before surgery,S-CAI scores were obviously lower in observation group than in control group [(14.05±2.32)points vs(16.75±2.33)points],which showed significant difference(t=9.00,P〈0.05);systolic blood pressure,diastolic blood pressure and heart rate were(17.6±2.5) mm Hg,(11.6±1.1)mm Hg(1 mm Hg=0.133 kPa)and(88.7±11.6) /min]in observation group,which were obviously lower than those in control group[(19.8±4.1),(14.2±2.1)mm Hg,(110.2±8.7) /min],and significant difference was found between the groups(t=5.02,12.01,16.24,P〈0.05).Observation group had higher satisfaction than control group,which showed significant difference(χ2=37.42,P〈0.05).Conclusions The implementation of seamless care model,perioperative centralized care management and humanistic care interventions can reduce fear,anxiety and other emotional responses,improve the sense of security and patient satisfaction.
