术前平均住院日影响因素分析

Preoperative Average Length of Stay and Influential Factors in a Certain Hospital from 2008 to 2012

目的探讨影响术前平均住院日的主要因素及降低患者术前平均住院日的有效途径。方法对某医院2008/2012年住院患者术前平均住院日及相关指标进行描述性统计分析,对其对患者医疗费用的影响进行相关分析,对术前住院日各影响因素进行多元回归分析。结果医院术前平均住院日呈逐年下降趋势。患者年龄、性别、职业、付费方式、手术类型等因素,患者等待检查、检验和手术时间以及医务人员意识对术前住院日有重要影响。结论平均住院日受多种因素影响。加强医护人员教育,优化术前检查和手术流程,实行预住院制度,提高疑难病例确诊率等,有助于缩短术前平均住院日。

Objective To investigate the main influential factors and explore the efficient methods to reduce preop- erative average length of stay. Methods The preoperative average length of stay of the inhospital patients from 2007 to June 2012 and the related indicators were analysed by descriptive statistical methods. Their relation with medical expenses was analysed by linear correlation method. Multiple regression analysis was performed to explore the influential factors. Results The preoperative average length of stay in the hospital was decreased year by year. Patients＇ age, sex, occupa- tion, payment kinds, of surgery type, patients＇ waiting time for examination, test and operation time and medical person- nel＇s consciousness can significantly affect the preoperative average length of stay. Conclusion The preoperative average length of stay is affected by multiple factors. Strengthening education of medical staff, improving preoperative examina- tion and surgery process, implementing prehospital system, increasing the diagnosis rate of difficult cases contributes to shorten the preoperative average length of stay.
