纳美芬联合甲氯芬酯对老年患者全麻下妇科肿瘤根治手术后苏醒的影响

Effect of nalmefene combined with meclofenoxate on recovery from general anesthesia in the elderly patients undergoing radical gynecological tumor surgery

目的观察纳美芬联合甲氯芬酯对老年患者全身麻醉下行妇科肿瘤根治手术后苏醒的影响。方法将60例择期行妇科肿瘤根治手术的老年患者随机分为A、B、C 3组,各20例。A组静脉滴注甲氯芬酯0.25g,B组静脉注射纳美芬0.25μg/kg,C组为联合使用纳美芬0.25μg/kg和甲氯芬酯0.25g。分别于术后0、5、10、20、30、60min记录各组MAP、HR、SpO2。记录各组术后睁眼时间、拔除气管导管时间、术后1h内恶心、呕吐、伤口疼痛的发生情况。结果在睁眼时间与拔除气管导管时间上,与A组比较,B组和C组的时间减少,差异有统计学意义(P<0.05)。与B组相比较,C组时间缩短,差异具有统计学意义(P<0.05)。各组MAP、HR、SpO2及术后恶心、呕吐、伤口疼痛的发生率差异均无统计学意义。结论纳美芬和甲氯芬酯联合使用可缩短老年患者全身麻醉下妇科肿瘤手术后苏醒时间。

Objective To study the effect of nalmefene combined with meclofenoxate on recovery from general anesthesia in the elderly patients undergoing radical gynecological tumor surgery. Methods 60 elderly patients undergoing radical gynecological tumor surgery were randomly divided into 3 groups： group A （n = 20, received meclofenoxate 0. 25g） ; group B （n = 20, received ualmefene 0.25μg/kg） ; group C （n= 20, received nalmefene 0. 25 μg,/kg and meclofenoxate 0. 25 g）. MAP, HR, SpO 2 were recorded at 0min, 5min, 10min,20min,30min,60min after operation. Wake up and extubation time were recorded. The adverse effects were also evaluated. Results Compared with group A, the wake up and extubation time were decreased both in group B and group C. The wake up and extubation time were shorter in group C than in group B. But there was no significant difference in MAP, HR, SpO2 and incidence of adverse reactions in 3 groups. Conclusion Nalmefene combined with meclofenoxate may shorten the time of recovery from general anesthesia in elderly patients undergoing radical gynecological tumor surgely.
