无创机械通气治疗慢性阻塞性肺疾病呼吸衰竭的临床观察

Clinical observation of noninvasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease and respiratory failure

目的探讨无创机械通气治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法将68例COPD合并Ⅱ型呼吸衰竭患者随机分为对照组和观察组,每组34例。对照组用常规药物治疗;观察组用常规药物加双水平正压通气(BiPAP)无创机械通气治疗。结果实验组呼吸频率(RR)、心率(HR)、动脉血pH、氧分压(PaO2)、二氧化碳分压(PaCO2)均明显改善。对照组血气分析变化不明显。结论无创机械通气治疗慢性阻塞性肺疾病急性加重期并发Ⅱ型呼吸衰竭可明显升高PaO2、降低PaCO2,改善机体低氧及酸中毒,是一种安全有效的治疗方法。

Objective To investigate the clinical efficacy of non-invasive mechanical ventilation in the treatment of COPD combined with type II respiratory failure. Methods 68 cases were randomly divided into the control group and the observation group, 34 cases in each. The control group was treated using conventional drug treatment, and the observation group was treated with conventional drugs plus BiPAP noninvasive mechanical ventilation therapy. Results The RR, HR, arterial pH, PaO2 , PaCO2 were significantly improved in the observation group after treatment. But there was no significant difference in blood gas analysis in the control group. Conclusion Noninvasive mechanical ventilation in the treatment of acute exacerbation of COPD period concurcent II respiratory failure can be significantly increased PaO2, lower PaCO2, to improve hypoxia and acidosis, which is a safe and effective treatment method.
