负压封闭引流术(VSD)治疗胫腓骨骨折内固定术后感染疗效探讨 被引量:7

摘要 目的比较分析持续性封闭负压引流术对于胫腓骨骨折内固定术后感染的临床疗效。方法回顾性分析1999年3月~2012年3月在我院治疗的胫腓骨骨折内固定术后感染病例。其中12例采取手术清创后局部换药的治疗方式,17例患者采取手术清创后持续性封闭负压引流治疗。结果两组患者获得9~28个月(平均13个月)的随访,负压引流组患者15例(88.2%)获得痊愈。无l例患者需要再次手术清创,骨折平均愈合时间5.5个月;局部换药组患者8例(66.7%)获得痊愈,3例患者进行再次手术清创并冲洗引流后获得临床治愈,骨折平均愈合时间6.8个月。两组患者在感染治疗的有效率及骨折愈合时间方面差异均有统计学意义(P〉0.05)。结论持续性封闭负压引流术对于胫腓骨骨折内固定术后感染的治疗具有操作简单、疗效可靠、功能恢复良好、创伤小等优点,值得在临床中广泛应用。 Objective To compare the analysis of closed suction drainage for the persistence of tibia and fibula fracture fixation postoperative infection: a clinical observation. Methods A retrospective analysis of 1999 March - 2012 year in March in our hospital for treatment of tibial and fibular fractures after internal fixation of infection cases. Among the 12 patients taking operation 'after debridement and local change medicine treatment, 17 patients by operation after debridement of persistence of vacuum-assisted closure therapy. Results Two patients received 9 - 28 months ( mean 13 months) of tollow-up, negative pressure drainage in 15 patients(88.2% ) was recovered, no 1 patients needed another operation debridement, fracture healing time averaged 5.5 months; changing dressings in 8 patients (66, 7% ) cured, 3 patients again operation debridement and irrigation and drainage obtained after clinical cure, fracture healing time averaged 6.8 months. Two groups of patients in the treatment of the infection rate and fracture healing time difference has statistics significance (P 〉 0.05). Conclusion The persistence of closed suction drainage for tibial and fibular fracture internal fixation on the treatment of infection after has the advantages of simple operation, reliable effect, good functional recovery, less trauma and other advantages, it is widely used in clinical.
