米力农联合酚妥拉明治疗危重型手足口病的疗效观察

Curative effect of milrinone combined with phentolamine in treatment of severe hand-foot-mouth disease

目的观察米力农联合酚妥拉明治疗危重型手足口病的疗效,以提高患儿预后。方法分析ICU收治住院的危重型手足口病(Ⅲa期)患儿68例,随机分为对照组与观察组,对照组给予米力农,观察组给予米力农联合酚妥拉明,比较两组疗效。结果治疗后1h,两组患者的心率、收缩压和左室射血分数(EF)以及心输出量(CO)较治疗前有明显的改善,而且观察组的改善效果明显的优于对照组,差异有统计学意义(P<0.05);观察组的末梢循环改善时间、心率、血压恢复正常时间分别为(2.47±0.27)、(65.76±11.09)、(66.35±8.64)h,而对照组的末梢循环改善时间、心率、血压恢复正常时间分别为(8.17±2.21)、(80.17±14.10)、(74.12±4.25)h,两组的数据比较差异有统计学意义(P<0.05)。结论米力农联合酚妥拉明能显著改善手足口病危重症患儿的生命体征,疗效显著。

OBJECTIVE To observe the effect of milrinone combined with phentolamine in the treatment of severe hand-foot-mouth disease so as to improve the prognosis. METHODS A total of 68 cases of children with critically severe hand foot and mouth diseases （Ⅲ a）, who hospitalized the ICU, were enrolled in the study and were ran-domly divided into the control group and the observation group. The control group was given intravenous milri-none, and the observation group was given milrinone combined with phentolamine, then the curative effect was compared between the two groups. RESULTS One hour after the treatment, the heart rate, systolic blood pres-sure, left ventricular ejection fraction （EF）, and cardiac output （CO） of both groups were significantly improved, and the observation group improved more significantly than the control group, the difference was statistically sig-nificant （P〈0.05）. The peripheral circulation improving time was （2.47±0.27） hours in the observation group, （8.17±2.21）hours in the control group; the tim of abnormal heart rate recovering to normal was （65.76±11.09） hours, （80.17±14.10） hours in the control group; the time of blood pressure returning to normal was（66.35 ± 8. 64）hours in the observation group, （ 74.12 ± 4.25） hours in the control group, the difference in the data between the two groups was statistically significant （P〈0.05）. CONCLUSION Milrinone combined with phentolamine can significantly improve the vital signs of the children with critically severe hand foot and mouth diseases, with the curative effect significant.
