推拿结合康复训练对全髋关节置换术后功能恢复效果的病例对照研究

Case-control Studies on Massage Combined with Rehabilitation Treatment for Functional Recovery of Total Hip Arthroplasty

目的:探讨推拿结合康复治疗对全髋关节置换(THA)术后恢复的临床疗效。方法:选择入住本院首次接受THA的患者60例,其中男37例,女23例;年龄45～76岁,平均56.8岁;住院时间15～25 d,平均l9.5 d;术前Harris评分12～43分;随机分为研究组与对照组各30例。两组在性别、年龄、假体固定方式及术前Harris评分等方面比较,差异均无统计学意义(P>0.05),术后对照组采用常规康复训练,研究组采用推拿结合常规康复训练,使用Harris评分、术后并发症发生率(疼痛、关节粘连、下肢深静脉血栓)作为评定指标,观察临床疗效。结果:研究组在术后第1周Harris评分与对照组比较,差异无统计学意义(P>0.05),在术后第2、4周Harris评分明显高于对照组(P<0.05)。研究组术后第2、4周的术后并发症发生率(疼痛、关节粘连、下肢深静脉血栓)显著低于对照组(P<0.01)。结论:推拿结合康复训练能明显减少THA术后并发症,对THA术后关节功能恢复有较好的效果。

Objective ：To evaluate clinical effects of total hip arthroplasty （THA） with the treatment of the massage combined with rehabilitation. Methods：From January 2010 to June 2012,60 patients with total hip arthroplasty were divid- ed into two groups according to visiting sequence, including 37 males and 23 females, ranging in age from 45 to 76 years old,with an average of 56.8 years old, the hospitalization time was 15 - 25 d, with an average of 19.5 d. Preoperative mean Harris score was 12 -43.60 subjects were randomly divided into the research group（ n = 30）and （n = 30）. There was no statistical significance in gender, age, prosthetic fixation and preoperative Harris score （P 〉 0.05 ）. Patients in the eontrol group were given routine rehabilitation training, the research group were given massage combined with rehabilitation training,and Harris score,the postoperative morbidity rate（ pain,joint adhesion, deep venous thrombosis）were as indexes for evaluating the. Results ： There was no statistical significance between research group and the control group in Harris score at first week （ P 〉 0.05 ）. Research group was superior to control group in Harris score at second week and fourth week （ P 〈 0.05 ）. The postoperative morbidity rate in research group was significantly higher than those in control group（P〈 0.01 ）. Conclusion： Massage combined with rehabilitation treatment can effectively reduce the incidence of postoperative complications, and has good effect on improving the recovery of function after THA.
