中医药在冠心病二级预防中应用现状的调查

Survey of TCM application status on secondary prevention of coronary heart disease

目的了解中医药在冠心病二级预防中的应用现状。方法采用统一调查表,调查在北京地区四所三甲医院就诊的526例急性冠脉综合征患者发病前6个月、发病时、发病后6个月内、发病后7～12个月内中医药的应用情况。结果在中医院就诊的患者4个时期中药使用率分别为27.1%、97.7%、78.4%、42.9%,在西医院就诊的患者中药使用率均低于10%。使用最多的中药静脉制剂、中成药、中药汤剂均为活血化瘀类。在没有应用中医药的原因中,52.3%～75.5%为医生没有此类医嘱,55.7%的患者没有接受过中医防治心脑血管病健康教育。结论在冠心病二级预防中,西医院的中医药使用率低,中医药在中医院使用较规范,中医药防治心脑血管健康教育工作亟待加强。

Objective To investigate the application status of traditional Chinese medicine（TCM） for coronary heart disease.Methods Using unified questionnaire,five hundred and twenty-six cases of acute coronary syndrome from four grade A-class 3 hospitals in Beijing were investigated for TCM application situation,at 6 months prior to the onset,during the period of the onset,within 6 months and 7-12 months after the onset of disease.Results The TCM usage frequencies were 27.1 %,97.7 %,78.4 % and 42.9 % respectively during the four periods in TCM hospitals,while the frequencies were all below 10 % in the Western medicine hospitals.The most common ones of TCM intravenous preparation,Chinese patent medicines and decoctions were all belong to blood-activating and stasis-resolving medicine.The reasons why Chinese medicines were not used were that there was no doctor＇s advice accounting for 52.3 %-75.5 %,and 55.7% of the patients didn＇t receive health education on TCM preventing and treating cardiovascular and cerebrovascular disease.Conclusion On the secondary prevention of coronary heart disease,the usage frequency is low in the Western medicine hospitals while the Chinese medicine is normatively used in the TCM hospitals.It should be urgently strengthened for the health education about TCM preventing and treating cardiovascular and cerebrovascular diseases.
