微创手术与开颅手术治疗高血压脑出血的对比分析

目的比较微创穿刺血肿清除术与小骨窗开颅显微手术血肿清除治疗高血压脑出血的临床效果。方法将68例高血压脑出血患者随机分为观察组和对照组，观察组采用微创穿刺血肿清除术治疗，对照组采用小骨窗开颅显微手术血肿清除治疗，比较手术时间、术中出血量、住院时间及治疗效果、生活质量改善情况。结果观察组术中出血量、患者住院时间少于对照组（P〈0.05）；手术时间差异无统计学意义；观察组Barthel评分高于对照组（P〈0.05）、神经功能缺损评分优于对照组（P〈0.05），观察组总有效率高于对照组（P〈0.05）。结论微创颅内血肿清除术治疗高血压脑出血，血肿清除效果好，对患者损伤小，有利于术后恢复。

Objective To compary the clinical effect of the minimally invasive puncture operation with small bone window with craniotomy in the treatment of hypertensive cerebral hemorrhage microsurgery. Methods 68 patients with hypertension cerebral hemorrhage patients were randomly divided into control group and observation group. The control group used small bone window craniotomy microsurgery treatment, the observation group used minimally invasive surgical treatment. The treatment effect, improvement of the quality of life, operation time, intraoperative amount of bleeding, length of hospital stay were analysed of the two groups after treatment. Results The observation group Barthel score was significantly higher than that in the control group, the difference was statistically significant （ P〈0.05 ） . The total effective rate of observation group was significantly higher than that in the control group, the difference was statistically significant （P〈0.05） . The nerve function defect score in observation group was significantly superior to control group （ P〈0.05 ） . During the operation, the amount of bleeding and hospital stay in the observation group were significantly less than in control group, with significant difference （ P〈0.05 ） . The observation group and control group operation time difference was not statistically significant （ P〉0.05 ） . Conclusion Minimally invasive intracranial hematoma removal technique in the treatment of hypertensive cerebral hemorrhage, hematoma removal effect is good with a little injury, which will help patients recover and is worth to application at clinical.
