MRI在急性膝关节前交叉韧带断裂的应用价值 被引量:4

摘要 目的探讨急性前交叉韧带(ACL)断裂的MRI特征及临床应用价值。方法回顾分析经关节镜证实的108例急性膝关节韧带外伤患者的MRI资料,分析其MRI表现,判断有无急性ACL断裂。结果急性ACL断裂患者具有ACL形态、信号异常等明显直接征象与关节内半月板损伤、骨挫伤等间接征象。参照关节镜检查,MRI诊断膝关节前交叉韧带前内侧束断裂敏感度100%,特异度93.3%,准确度94.4%;外侧束断裂敏感度100%,特异度97.9%,准确度98.1%;完全断裂敏感度94.6%,特异度100%,准确度96.3%。结论 MRI诊断急性ACL断裂具有很高的准确性,MR是急性前交叉韧带断裂临床首先检查方法。 Objeetive To analysis the evaluation of MRI diagnosis of anterior cruciate ligament (ACL) tears. Methods MR/appearances of injured knee ligament in 108 cases were relrospeetively reviewed. Analysis of the MRI findings to determine the presence or absence of acute rupture of the ACL. Results Patients with acute rupture of the A CL morphology, signal abnormalities apparent the direct signs and articular meniscus injury, bone contusion of indirect signs. Reference of knee arthroseopy standard, MRI diagnosis of knee anterior eruciate ligament, the anteromedial bundle fracture sensitivity 100%, specificity 93.3%, accuracy 94.4%; lateral beam sensitivity 100%, specificity 97.9%, accuracy 98.1%; complete rupture sensitivity 94.6%, specificity 100%, accuracy 96.3%. Conclusion MRI diagnosis of acute ACL rupture with a high degree of accuracy, MR is the first inspection methods of acute rupture of the ACL.
