(1,3)-β-D葡聚糖检测对AIDS患者真菌感染诊断的临床意义 被引量:4

摘要 目的探讨(1,3)-β-D葡聚糖检测对获得性免疫缺陷综合征(AIDS)患者真菌感染诊断的临床意义。方法收集116例AIDS患者的血液、脑脊液、深部咳出的痰液以及其他深部组织液进行真菌培养,采用法国生物梅里埃微生物快速鉴定(API)/ATB自动细菌鉴定仪及其配套试剂进行真菌鉴定。用MB-80微生物快速动态检测系统进行(1,3)-β-D葡聚糖的检测。结果 116例AIDS患者中真菌培养检出78例真菌,分别是18例念珠菌,44例马尔尼菲青霉菌,16例新型隐球菌;这78例真菌用(1,3)-β-D葡聚糖检测,念珠菌、马尔尼菲青霉菌、新型隐球菌阳性分别为12、33、9例。8例痰液培养出念珠菌,但是其(1,3)-β-D葡聚糖浓度均小于20.0pg/mL。有8例真菌培养阴性,而(1,3)-β-D葡聚糖检测呈阳性;重新检测后,结果依然。14例真菌培养阴性,而(1,3)-β-D葡聚糖检测呈阳性;重新检测后,(1,3)-β-D葡聚糖检测为阴性,真菌培养仍为阴性。真菌培养念珠菌、马尔尼菲青霉菌、新型隐球菌阳性标本及健康对照组标本进行(1,3)-β-D葡聚糖检测,(1,3)-β-D葡聚糖浓度分别为(310.22±143.23)、(530.61±263.24)、(50.15±22.33)、(4.00±1.52)pg/mL,前三者与健康对照组比较,差异有统计学意义(t值分别为42.26、65.38、18.65,P<0.05)。结论 (1,3)-β-D葡聚糖检测和真菌培养可以互为补充,(1,3)-β-D葡聚糖检测是诊断AIDS患者真菌感染的有效辅助手段。
